How the conversion works

Once a peptide is reconstituted, its concentration (milligrams per millilitre) is fixed by how much peptide was in the vial and how much bacteriostatic water was added. That concentration is what lets you translate between a mass (mcg or mg), a volume (mL), and the units marked on a U-100 insulin syringe.

The two relationships

  • Mass to mass: 1 mg = 1,000 mcg. This never needs a concentration.
  • Mass to volume to units: volume = mass ÷ concentration, and on a U-100 syringe, 100 units = 1.0 mL. These need the concentration.

A note on “units” and “IU”

The “units” here are U-100 insulin-syringe units, a measure of volume where 100 units equals 1 millilitre. They are not International Units (IU), which are peptide-specific and are not convertible to a mass without a per-peptide factor. This converter deliberately does not convert IU, to avoid a misleading result.
